The CHSE Online Application for Program Accreditation

Guide book for the application process

Please complete and return this application at least one month prior to your program.

In order to apply for a Program Accreditation you are required to create a password and complete the following online form.

You must complete each item in order to proceed to the next section. Each section you complete (ending with the submit button) will be saved if you choose to log off the system and complete the application at a later date. Take note, progress of incomplete sections is NOT SAVED. Entries that are mandatory have a (*) symbol next to them. Applications must be completed within 30 days or they will be removed from the system.

Ethical Standards

 

Effective January 1st, 2007, our policy has changed to include the following:
We require an electronic copy (in Excel format) of your full registrant/attendee list with names, addresses, and professional designation with specialty provided. A paper copy of the final attendee list with signatures should be mailed to our office no later than four (4) weeks after your event. This registration information will be retained by the Continuing Health Sciences Education Program for audit purposes and in compliance with the Freedom of Information and Protection of Privacy Act (FIPPA). 


The non-refundable fee for accreditation application review is $300
and an additional fee of $15 per event registrant/attendee if the lists are provided in electronic format otherwise the fee is $25 per event registrant/attendee, payable to McMaster University. Fees may vary based on educational programs being developed in partnership with another professional organization, government agency or independent communication company. Please contact the CHSE office if your require further information.

Please complete this online application at least one month prior to your program. When you have completed the online application, you will be presented with a PDF that must be printed and signed. Please fax the printed document with the Planning Committee's Chair's signature to the attention of:
